Eligibility for services is determined through a pre-screening process. Requirements may vary depending on the type of care needed.
Who May Qualify
You may qualify for services if you:
- Are uninsured or underinsured
- Meet income guidelines (up to 250% of the Federal Poverty Level)
- Are in need of breast health services
- If diagnosed with cancer, are in Stage III or earlier
- Live within our 28-county North Texas service area (see below)
Program Requirements
The following guidelines help determine eligibility and access to services:
- Eligibility is determined on a case-by-case basis
- Individuals may be referred to State, County, or other programs
- Services are provided based on available funding
Important to Know
Please review the following before applying:
- A $20 application processing fee is required
- Eligibility must be recertified every 6 months
- Individuals receiving treatment services contribute $1,000 per phase of care (surgery, chemotherapy, radiation)
