How the Process Works

Follow these simple steps to get connected to care:

  2. Eligibility Review - We review your information to determine what services or programs you may qualify for.  Check your Eligibility
  3. Complete Required Forms - If eligible, you will be guided to complete and submit the appropriate application.
  4. Get Connected to Care  - Approved clients are scheduled with a trusted provider within our network, and our team will guide you through the next steps in your care.
